ATHLETE ELIGIBILITY FORMS

Welcome to the new athletic paperwork process for your student-athlete. ALL student-athletes must complete this new process and be cleared prior to participation for in-season conditioning, tryouts, practices, and athletic contests as well as out-of-season conditioning.

Go to AthleticClearance.com and choose Florida.

  1. PARENTS register with a valid email username and password.

  2. Login using the email address that you registered with.

  3. Select Start Clearance” to start the process.

  4. Choose the School Year: 2023-2024.

  5. Choose our School: Highlands Christian.

  6. Choose Sport (pick the sport your child wants to participate in for the fall, winter, or spring season. You can add additional sports later).

  7. Complete all required fields for Student Information, Educational History, Medical History, and Signature Forms. Note: During this process, you will confirm that your student-athlete has watched the 3 NFHS Learn Courses (Concussion in Sports, Heat Illness Prevention, & Sudden Cardiac Arrest). These courses are available here.

  8. The following forms need to be printed, filled out, and uploaded during this process. *Recommendations on how to upload documents are below.

    1. “Physical Form” aka FHSAA EL2 physical will need to be uploaded. Athletic Clearance website will say “Physical Form” and that is when you will upload the FHSAA EL2 (all three pages as one document *instructions on how to do this at the bottom of this document).

    2. “Proof of Insurance” will need to be uploaded. That is where you will upload a picture of your health insurance card. Health insurance is mandatory by the FHSAA in order to play sports in Florida.

    3. “Upload Concussion Certification” and “FHSAA Required Video Certificate” will be used to upload an NFHS course certificate from completing the required video courses in #8 above (or an additional form option if needed).

    4. “Additional Form” may be used to upload the GA4 form, any of the homeschool forms, or page 2 of the EL2 form if uploading later.

  9. Once you have completed the required fields and uploaded the required forms you will be returned to the ‘clearances’ page where you will be able to view the status of your submission. You will begin with an ‘uncleared’ indicator. Once your submission has been reviewed and approved by the athletic office your status will change to a ‘cleared’ indicator. It is at this point that your student is ready to participate in athletics.

RECOMMENDATIONS FOR #9: Forms to be printed and uploaded:

  1. Physical Form” – print the FHSAA EL2 pages 1-3. Fill out completely, have the physician sign and date. Upload to your computer. Additional Form” – print the GA4 page (and EL7 & 9 if you’re a homeschool student), fill out, and upload to your computer as ONE document (instructions below).

  2. Please do NOT use the documents library feature rather use the choose file” button.

  3. We recommend that you download the Genius Scan app (below) to your phone for scanning your EL2 and uploading it to your computer.

*If you do not know how to upload a document to your computer or merge multiple pages into a single document, we would recommend the following:

  1. Print the forms you need and have them completely filled out. Download the ‘Genius Scan’ app to your phone.

  2. Using the ‘Genius Scan’, take photos of the completed form combining into one pdf file (for the FHSAA EL2 do not forget doctor’s signature!).

  3. Follow prompts on ‘Genius Scan’ to email pdf file to your personal email account.

  4. Log in to your email account on your desktop / laptop computer.

  5. Open email from ‘Genius Scan’ and save pdf file attachment to your desktop.

  6. Upload it to Athletic Clearance when prompted on the ‘Step 1-Student Information Uploads’ page by clicking on the ‘choose file’ and selecting the pdf file from your desktop.

+ FHSAA

Highlands Christian Academy is a member of the FHSAA.

+ Academic Eligibility

  • Students in grades 9-12 must have a cumulative 2.0 unweighted GPA on a 4.0 scale to remain eligible.
  • Students in grades 6-8 must have been regularly promoted from the previous grade, carrying a normal class load, and maintaining a 2.0 GPA.

+ Limits of Eligibility

  • Students have four consecutive years of eligibility, beginning when they first enter the 9th grade.
  • Four years after a student enters the 9th grade, that student shall become ineligible.
  • Junior high students may participate in interscholastic athletics one year as 8th grader, one year as 7th grader, and one year as 6th grader.
  • A student who reaches the age of 19 prior to September 1st shall become permanently ineligible.

+ Transfer and Eligibility

If a student transfers from another school to Highlands Christian Academy and has participated in a sport during the same school year, that student must be authorized for Good Cause in order to be eligible in that sport. This applies to homeschool students as well. Good Cause Policy: Student must go before the Secondary Principal, explain the reason for transferring, and be approved to participate immediately.

+ Homeschool Students

Qualified homeschool athletes may participate in the athletic program at the discretion of the administration. All homeschool applicants must fill out the Highlands homeschool student information card. Homeschool athletes must comply with the rules and standards of Highlands. They must also comply with the FHSAA rules regarding homeschool programs. These forms are available for download in the “FHSAA Forms” tab or in the Athletic Office.

+ FHSAA Forms

In accordance with the bylaws of the FHSAA, each student must have a current physical examination, which is good for one calendar year. No student may try out for a sport or participate in any way until he or she has had a physical exam. Players and parents must also fill out a Consent and Release Certificate.

+ Athletic Fees

Each student who participates in the athletic program at Highlands is required to pay an athletic fee. These fees vary depending on the number of sports played. The due dates will be September 15th for Fall sports, December 15th for Winter sports, and February 15th for Spring sports.

+ Highlands Athlete Fees

Elementary

  • First Sport – $100
  • Second Sport – $50

Secondary

  • First Sport – $200
  • Second Sport – $100

+ Homeschool Athlete Fees

  • Elementary – $250
  • Secondary – $400

+ Individual Team Dues

There may be additional fees per team. These fees include but are not limited to personal equipment, travel costs, etc. This will be communicated by each coach to the parents if there are any extra fees per sport. Fundraising options may be available.

+ Admission Prices

Entrance to regular season home games at Highlands are $5 for adults and $3 for students. Highlands staff and students enter free. Season passes can be purchased. Admission prices for District and Regional games are set by the FHSAA.
